
WHAT IS IBD R.E.D.I.?
IBD R.E.D.l. is a free, standardized toolset for teens and young adults with IBD and their healthcare providers to use to help prepare for the transfer from pediatric to adult care. The tools are designed to efficiently reveal areas in IBD-related knowledge or skills that need to be addressed as teens and young adults prepare for transition, and to allow the TBD care team to provide personalized patient education and skills training.
The IBD R.E.D.I. Program toolset was developed in collaboration with Improve Care Now and supported by an unrestricted educational grant from Pfizer. Inc. Modules were created with guidance from an advisory team comprised of IBD healthcare providers, people living with IBD and a parent.
Knowledge Assessments
Goal: To help the person with IBD understand their level of IBD knowledge in various aspects of IBD care and the IBD care team to provide targeted education.

Responsibilities Checklists
Goal: To help the patient gradually transition responsibility for IBD care in the home care setting and to provide the IBD care team with effective, evidence-based resources to track transition readiness.
